Case Management

Petaluma People Services Center Case Management Program helps older adults (60+) maintain independence, safety, and quality of life while aging in place. Our Case Managers provide personalized support, helping clients navigate challenges, access resources, and connect with services that support their overall well-being.

How We Help

Services may include:

  • Individualized assessments and care planning

  • Resource navigation and referrals

  • Benefits and service coordination

  • Advocacy and problem-solving support

  • Connection to nutrition, transportation, caregiver support, counseling, and other community resources

  • Ongoing support as needs and circumstances change

Supporting Emotional Well Being

Emotional health is an important part of healthy aging. As part of our Case Management Program, clients are screened for symptoms of depression and may be offered Healthy IDEAS (Identifying Depression, Empowering Activities for Seniors), an evidence-based program designed to support emotional well-being.

Healthy IDEAS helps older adults:

  • Recognize symptoms of depression

  • Connect with healthcare and mental health resources

  • Increase engagement in meaningful activities

  • Improve overall well-being and quality of life

Healthy IDEAS is integrated into case management services, providing additional support when needed.
