Case Management

Petaluma People Services Center Case Management Program provides person-centered assistance to older adults (age 60+) who are no longer able to manage daily living tasks by helping them maintain an optimum level of functioning in the least restrictive setting as possible.

• Are you or a loved one at risk of declining over all health, becoming depressed, and or losing independence because of diminished social and/or mental stimulation?

• Are you or a loved one low-income, minority, Spanish speaker, socially isolated, reside in a rural part of the count and/or at risk of institutional placement?

Contact our Case Management team to get a full assessment of service that may be available to you. They will work with you and your family develop a care plan that includes specific goals that address identified needs and strategies.
