Aging Well Together — Case Manager

As a Case Manager for our Aging Well Together Department, you will connect seniors with services and empower them to maintain a healthy independence. You will work with clients and their family to counsel and aid older adults, age 60 or older, who need assistance managing daily living tasks in order to maintain optimum levels of functioning in the least restrictive setting possible. This is done through assessing individual cases and developing a care plan that includes nutrition options, transportation referrals, and other supportive services. You will work in partnership with our entire Case Management and Senior Services team of employees and volunteers.
